ProAct is focused on chronic disease management.  At ProAct, we believe that for the foreseeable future the greatest improvements in chronic disease outcomes and economics will result from strategies that incorporate the technology enabled delivery of integrated solutions using existing device technologies to repair broken outdated supply chains.  Chronic diseases are generally treated in a fragmented environment that includes many disparate providers delivering components of care rather than a complete solution for the condition.  ProAct has developed a proprietary cloud-based platform, the iSync™ System, that enables the delivery of chronic disease care within a virtual delivery infrastructure.  ProAct’s Infrastructure as a Service (IaaS) represents the future of people/patient management with innovative clinical protocols, process flows, integrated patient care, and inter-provider and patient communications through a range of applications operating within the iSync™ System.  The core of the iSync™ Platform consists of a HIPAA compliant healthcare database and an extensive and granular set of business services designed using a homogenous technology stack. These foundational elements feed into a business design manager and then to a workflow management system, which enables the Company to create state-of-the-art healthcare solutions with minimal coding requirements.   The System can be deployed on the cloud or on premise.  This architecture enables ProAct to develop scalable solutions that bring together disparate providers to manage the total “continuum of care” for a specific disease.  ProAct has completed development and is beginning its rollout of a nationally scalable solution for Obstructive Sleep Apnea (OSA) and will follow with solutions aimed at better management of some of its comorbid conditions, including obesity and diabetes where the incidence of OSA can exceed 50%.
