Farzad Mostashari
Founder, Aledade Inc former National Coordinator for Health Information Technology
Farzad Mostashari, MD, ScM is the co-founder and CEO of Aledade, Inc., a company he established in June 2014.13 Aledade is the largest network of independent primary care practices in the United States, serving more than 1,500 practices across 45 states and being accountable for the care of over two million patients.1
Dr. Mostashari has an extensive background in healthcare policy and health information technology. He previously served as the National Coordinator for Health IT at the Department of Health and Human Services from April 2011 to October 2013.3 Before that, he was a distinguished expert at the Brookings Institute's Engelberg Center for HealthCare Reform and founded the NYC Primary Care Information Project.12
His educational background includes:
