Daniel J. Chavez is the Executive Director of the Serving Communities Health Information Organization (SCHIO), having taken on this role in March 2022. He is notable for being the first person to hold this position at SCHIO, which is one of the oldest health information exchanges in the United States, serving the Santa Cruz community and surrounding areas.

Chavez has over 35 years of experience in health care information technology (HIT). Before joining SCHIO, he was the founding executive director of San Diego Health Connect, a health information exchange serving San Diego and Imperial Counties.
